ProSpine Itemized Billing?

This isn't exactly a travel question, but I figured that the posters here might have some answers.

For patients getting surgery with ProSpine and hoping to file insurance claims for reimbursement, what's the best way to get an itemized bill? ProSpine uses a "single price" approach for everything including facility, implant, surgeons fee, anesthesiologist, etc. Insurance companies want separate bills for all these services as well as lots of detail within each category.

Does anyone how who at ProSpine would best understand how to get this kind of cost breakdown?
