3-4 level ADR Lumbar Spine

Hi,
I had a 2 level at L4 & L5 ADR 7 years ago and was doing great until I had a bad fall...Now I'm having right butt/back pain...Facet blocks didn't work..Can't sit...
Anyone get 3 level or 4 level ADR??? Thought the most they could do was a 2 level??
