Cycle, sorry for the late reply.

I have seen many 3-level lumbar surgeries. I know people who have had them done in the US and overseas.

When a surgeon tells you that three-level ADR's cannot be done, they are not telling the truth. Maybe HE (or she) can't do them. Maybe it's because the risk management department at their hospital won't allow them, or the manufacturer won't allow him to (at his experience level).
